Payment Payment is required at the time of service. Cash, check, most major credit cards, and HSA/FSA cards are accepted. When canceling or rescheduling an appointment, 24-hour notice is required to avoid payment equal to the cost of service.
Health Insurance Reimbursements My services may be eligible for out-of-network reimbursement from health insurance plans that include mental health benefits. I am happy to provide the information necessary for you to complete your insurance reimbursement paperwork and will discuss this with you in advance of starting service.
The following questions are provided as a courtesy for you to inquire regarding your insurance plans mental health benefits should you like to explore out-of-network reimbursement.
Do I have mental health benefits as part of my insurance/health plan?
Does my mental health coverage offer "out-of-network" reimbursement?
Does reimbursement require pre-approval or referral by my primary care physician?
Do I have a deductible? If so, how much is it and have I met it yet?
What are the limits of my coverage? (sessions per year, amount per session, etc.)
What is the "usual and customary rate" for each service? (In healthcare terms, an initial evaluation is coded as a 90791, a 45 min. therapy session is a 90834, a 60 min. therapy session is 90837, and family (or couple) therapy is 90847)
