The following policy documents provide further information about my services and how I run my practice. You will note information about both your rights and responsibilities. Please review this material before coming in for your first appointment. I will reserve time for questions in our first meeting. I ask you to acknowledge receipt and understanding of these documents and sign an agreement to comply with the Office Policies and Social Media Policy and provide informed consent for treatment. If you are not able to access these documents or others on this page, I will provide them to you at your first appointment (office) or in-person consultation (home-based) allowing you to review and complete all necessary paperwork prior to your first appointment. Please plan to arrive 20-30 minutes early to your first office appointment if you plan to complete your paperwork onsite.
